Partner Education Application Approval

The proposed education is considered to be directly related to the employee's work and/or health career plans and will be beneficial in the performance of their job. Approval requires that the partner has met expectations over the last 12 months and has received no corrective action. If you approve this partner's participation in the education please fill out the approval form below. With your approval, this education request will be submitted to the Education Approval Committee. You may be invited to the approval committee to provide additional information related to the education requested.
