Mercyhealth policy requires that all partners and affiliated staff, students, vendors/contractors, etc. receive an annual influenza vaccination. A religious exemption may be granted if the individual (i) Sincerely holds a religious beliefs against vaccination, (ii) completes this form, and (iii) provides documentation to support the exemption request. Mercyhealth is committed to providing a safe, inclusive, and supportive experience for all and recognizes sincere observance of faith as it pertains to the practice of vaccination.
Individuals with an approved exemption may be required to comply with other preventative requirements as specified in the exemption approval and as may be updated by later notification and/or posting of the requirements on the Mercyhealth Intranet. In the event of an outbreak on or near Mercyhealth campuses, individuals with exemptions may be excluded from facilities and activities, in order to protect all unvaccinated members of the Mercyhealth community, until the outbreak is declared to be over.
Religious exemption requests will be reviewed by the Senior Director of Human Resources, Vice President of People and Culture or designee. Although Mercyhealth carefully considers all requests for religious exemptions, approval is not guaranteed. You will be notified, by email, if your exemption is granted or denied. The decision is final and not subject to appeal. Individuals are permitted to reapply if new documentation or information become available.
